The primary goal of the reversal procedure is to carefully align the lumen of the two ends of the vas deferens and then achieve a water tight closure. To maximize the chances of success of the process, a sample fluid is taken from the testicular end of the vas deferens and this is examined under a laboratory microscope. This fluid ought to contain some sperm but there are three possibilities encountered when examining this fluid sample. The first and best situation is when the fluid is found to contain whole sperm. The second possible scenario, which is more common than the first scenario, is that that the fluid is thin and contains only sperm parts. The third possible situation is that the fluid is thick and pasty and contains no sperm at all.

In the third scenario, a vasoepididymostomy must be performed in addition to the vasovasostomy to remove the obstruction, which is known as an epididymal blowout. Both sites will need repair for a successful outcome but there will be no additional cost for the patient even if both repairs have to be done.
